The problem DOES appear to get worse the more people that are working at the same time - but that could just be that the error pops up fairly randomly, so it seems worse when more people are scanning just because of the small chance of it happening at all. Load is almost nothing - it isn't like the server is overworked at all. I've watched server traffic too and don't see anything that would be causing this. They can't figure out why it is happening.
However, Dexis/Dentrix have repeatedly verified that all of the software settings - including calibration files - are correct. (They have also tried various things to verify that it isn't hardware - including new powered USB hubs, extension cables, and so on.) Because it started suddenly with ALL of the sensors across ALL of the PCs, I really don't think it is a hardware issue. They get this behavior using 3 different sensors across 7 different PCs - so it does not appear to be hardware related. They can sometimes (but not always) click through the error and then manually advance through the rest of the scans. They will try to scan and then get an "Error 57: Failed to disarm." Sometimes they get it right away - but more often they get it after the 1st or 2nd scan while doing multiple scans. Somewhere around mid-January, they started having intermittent problems trying to use the Dexis sensors.
